Welcome to Case Log Pro ("Application," "Service," "we," "us," or "our"). These Terms of Service ("Terms") govern your access to and use of the Case Log Pro application, including our web application, mobile application, and associated services.
By accessing or using the Service, you agree to be bound by these Terms. If you do not agree to these Terms, you must not access or use the Service.
By creating an account, logging in, or using any feature of the Service, you represent and warrant that:
- You are at least 18 years of age or the age of legal majority in your jurisdiction
- You have the legal authority to enter into these Terms on behalf of yourself or your organization
- You are a licensed healthcare professional or authorized personnel with proper credentials to handle Protected Health Information (PHI)
- You have read, understood, and agree to our Privacy Policy
- You consent to our data practices as described in our Privacy Policy
IMPORTANT: You must accept both these Terms and our Privacy Policy before you can access Protected Health Information (PHI) within the Service. Annual re-acceptance of the Privacy Policy is required to maintain access.
2. Description of Service
Case Log Pro is a healthcare case logging and documentation application designed for medical professionals. The Service enables users to:
The Service is intended exclusively for use by physicians, medical residents, medical students, authorized observers, and healthcare administrators with proper credentials.
- Create, store, and manage medical case entries and logbooks
- Document surgical and procedural cases with patient-related information
- Organize cases by specialty, procedure type, and date
- Share and verify case documentation within authorized teams
- Access case data through web and mobile applications
3. Account Registration and Eligibility
To use the Service, you must create an account by providing accurate, current, and complete information. You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account.
The Service is designed for healthcare professionals and trainees. You agree to:
- Use strong, unique passwords
- Not share your credentials
- Notify us immediately of unauthorized access
- Log out from shared or public devices after each session
4. User Responsibilities
You agree to:
- Use the Service only in accordance with your authorized access and permissions
- Access only the minimum necessary information required for your duties (Minimum Necessary Principle)
- Not attempt to bypass or circumvent access controls
- Report any access or permission errors to your administrator
5. Acceptable Use Policy
You may use the Service only for lawful purposes related to:

- Share login credentials
- Export PHI outside the Service except as authorized
- Attempt to bypass security controls
- Introduce viruses or malware
- Disrupt the Service
- Misrepresent your identity or credentials
- Violate HIPAA or applicable regulations
Violation may result in immediate suspension or termination, revocation of access, reporting to your institution, reporting to regulatory authorities, or legal action.
6. Protected Health Information (PHI) and HIPAA
As a user handling PHI, you agree to comply with HIPAA and its implementing regulations, use the Service consistently with the HIPAA Privacy and Security Rules, maintain the confidentiality of PHI, and report suspected breaches immediately.
We implement safeguards including:
- Encryption of PHI at rest (AES-256-GCM)
- Encrypted transmission (HTTPS/TLS)
- Access control
- Audit logging
- Data retention and secure deletion
- Breach detection and notification
If you are a Covered Entity under HIPAA and will disclose PHI to us, a Business Associate Agreement (BAA) may be required. Contact us to execute a BAA before using the Service with PHI.
7. Intellectual Property
The Service, including its design, features, functionality, code, and trademarks, is owned by us or our licensors. You retain ownership of content you create. By submitting content, you grant us a limited license to store, process, display, and transmit that content to operate the Service.
8. Privacy and Data Protection
Your use is also governed by our Privacy Policy. You must accept the Privacy Policy before accessing PHI and re-accept it annually to maintain access.
9. Third-Party Services
The Service may integrate with third-party services (hosting, authentication, storage, email). Use is subject to their terms and privacy policies. We are not responsible for third-party practices.
10. Disclaimers
The Service is a documentation tool only. It does not provide medical advice, diagnosis, or treatment recommendations. THE SERVICE IS PROVIDED 'AS IS' AND 'AS AVAILABLE' WITHOUT WARRANTIES OF ANY KIND. We do not guarantee uninterrupted access.
11. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW:
- We shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including loss of profits, data, or goodwill
- Our total liability for any claims arising from or related to these Terms or the Service shall not exceed the amount you paid us in the twelve (12) months preceding the claim, or one hundred dollars ($100), whichever is greater
- We shall not be liable for any failure or delay due to causes beyond our reasonable control
- Some jurisdictions do not allow the exclusion or limitation of certain damages. In such jurisdictions, our liability is limited to the maximum extent permitted by law
12. Indemnification
You agree to indemnify Case Log Pro, its affiliates, officers, directors, employees, and agents from claims arising from:
- Your use of the Service
- Your violation of these Terms or any law
- Your violation of third-party rights
- Content you submit
- Any breach of HIPAA or privacy laws
13. Term and Termination
These Terms remain in effect for as long as you use the Service. You may terminate your account at any time. We may suspend or terminate access at any time for violation of Terms, non-payment, inactivity, or to comply with legal requirements.
14. Changes to Terms
We may modify these Terms at any time. We will notify you of material changes by posting the updated Terms, sending email, or displaying an in-app notice. Continued use after changes constitutes acceptance.
15. General Provisions
These Terms, together with our Privacy Policy and any applicable BAA, constitute the entire agreement. If any provision is invalid, the remaining provisions remain in effect. Our failure to enforce any right does not waive it. You may not assign these Terms without our consent.
